The Types of the trend of the course of Pulmonary Tuberculosis seen on the X-ray
ÀÌÂù¼¼/Chan s. Lee
Abstract
Summary;
There are three types of trend in the curse of pulmonary tuberculosis can be
classlfied according to their behavier observed on the serial X-ray films. Type I is the
one which showing the tendency of slight improvement soon after the treatment started
and remains with very few change for long period. Type ¥± is the one showing slow
progession of the disease with the occasional episodes of spread and death. Type ¥² is
the one showing rather rapid marked improvement soon after the treatment. then it
remains with very few change for long period. The frequency of the occurrence of the
type in each group of the patient Far Advanced, Moderately advanced, Mininmal, and
the relationship between the types and the durstion of the disease has been studied. In
the case of 120 pats. of F.A.40% of them belonged of the Type ¥°, 50% to the Type ¥±,
10% to the Type¥². There has been only Three Deaths seen in the period of more than
three years from type ¥±, it will make only 2.4% of the F.A. and thought to be the
effect on prolonging the life of the F.A. by. the Chemotherapy and Sanatoriurm Regime.
In the case of 150 pats. of M.A. 50% belonged to the Type ¥°, 20% to the Type ¥±,
30% to the Type ¥². There were only three cases of spread ; 2% of M.A. It can be
considered that the antimicrobial drugs minimised the spread of the disease but it is not
powerful enough to prevent all spreads. In the case of 30 pats. of Min. 78% belonged to
the Type ¥°, 2% to the Type¥±, 10% to the Type ¥². One particular case which
belonged to type ¥±was the one showed a cavity formation that disappeared in a shoot
period. The author can neither to say that the chemotherapy would not always effect to
prevent cavity formation nor to say that it always accelerate the healing process of the
cavity. There was a definite relationship between the type and the duration of the
disease ; the earlier to begin the treatment the more chance to get a marked
improvement in a short period. It was very similar in all of the group. The majority
duration of the disease prio to the treatment was 2 to 3 year among the Type ¥°, more
than 5 years among the Type ¥±, and less than I year among the Type ¥². (See table
1,2,3.) The materials were selected from the in-patients of the National Masan
Tuberculosis Sergery. They have been treated with Chrmotherapy alone or combined
with pneumoperitoneum without Sergery. The total number of observed was 300; 200
males, Ioo females. There were ,40% of F.A., 50% of M.A.,4., 10% of Min. The age
ranged from 17 to 55 and 78% of them were between 25 to 35 years. It was about 20%
who combined with Pneumoperitoneum and its duration was 6 months to 2 year. The
average total doses of antitubebulosis drugs were Streptomycin, 180gm., P A S, 8000gm,
I N H, 150§·. The formulars of the combination of these drugs were various as the
period of administration had been very long ; there were frequent tendencies of swiching
the drug by the order of the physician himself or by the patient¢¥s demand when it had
been tested without definit effect for some period, mostly about 6 months. The present
status of administration of these drugs are as follows, SH-PAS-lNN, 25%, INH-PAS,
25%, SM-PAS, 50%. The Duration of these observations was more than 3 to 5 years
since 1952.
